Help my sister in law is losing it?

It sounds like she had the baby before she was ready to accept the responsibility for him. Is there someone else who could take in the boy? A grandparent or other relative? It's my guess the boy's behavioral problems come from never being taught otherwise. Unfortunately, it'll take someone who knows how to set boundaries, provide discipline, but also provide a lot of love to help him get on the right track now.
